Health Forms and Immunizations 

To enroll at a university in the United States, all students must comply with medical standards set by the American College Health Association (ACHA). In accordance with the Center for Disease Control (CDC), Hiram College is required to have complete medical records and immunizations/tests mandated by U.S. government regulations. Therefore, students are required to do the following:

  1. Complete all four (4) pages of the Hiram College - History and Physical Exam - Immunization Record before arrival in the U.S. Visit your personal physician or clinic to obtain your medical history/exam and any immunizations you may need.
  2. Receive the three immunizations listed below if you do not already have them:
    • Two (2) MMR's (Measles, Mumps, and Rubella)
    • Tuberculosis Skin Test within the last 6 months (* BCG cannot be substituted for this test.)
    • Current immunizations for Diptheria, Tetanus, Polio
    • For added protection, the College strongly recommends (but does not require) vaccination for Hepititis B, Meningococcal, and Vericella/Chicken Pox (highly recommended for those who have NOT had the disease).
  3. Return the complete Hiram College - History and Physical Exam - Immunization Record

Students who do not return a fully completed Hiram College - History and Physical Exam - Immunization Record or receive all of the required immunizations by the start of the first semester at Hiram College will be required to obtain all immunizations/medical care at the Hiram College Health Center. Students will be notified of the day on which they are to receive immunizations. Expenses for these services will be billed to the student's account; specific immunization costs can be obtained from the International Student Advisor.
