Students With Disabilities


Students requesting accommodations must provide official documentation of a disability from a medical doctor, psychologist, or psychiatrist. This documentation must be no more than three years old. It should include the date of assessment and diagnosis, list assessment tools used and the results of these assessments, a description of past services, a description of the functional limitations of the disability. This documentation must support the need for specific accommodations requested based on the assessment and/or medical documentation. In some cases, a student’s most recent IEP will be sufficient documentation, however, Hiram College reserves the right to require that a student update or obtain more complete documentation before requests for accommodations will be considered. Official documentation should be forwarded to Dr. Lynn B. Taylor, Director of Counseling, PO Box 67, Hiram College, Hiram, OH 44234. Faculty members will receive a letter of accommodation each term for each student whose requests have been appropriately documented, reviewed, and approved. Any other students who indicate that they have a disability and are asking for accommodations in the classroom should be directed to Lynn for information. We can refer students to professionals in the area who can provide an assessment if there appears to be sufficient indication of a disability.
